**THERE WILL BE A $500 DEPOSIT COLLECTED AT TIME OF SCHEDULING FOR AUTISM EVALUATIONS. WE WILL NEED A 2 WEEK NOTICE OF CANCELATION IF YOU ARE NOT ABLE TO ATTEND YOUR SCHEDULED TESTING DATES, IF 2 WEEK NOTICE IS GIVEN FOR CANCELATION, REFUND OF DEPOSIT WILL BE ISSUED. THE $500 DEPOSIT WILL BE APPLIED TOWARDS PATIENT RESPONSIBILITY AFTER TESTING IS SUBMITTED TO INSURANCE, IF THERE IS NO PATIENT BALANCE AFTER INSURANCE PROCESSES THE CLAIM, IT WILL BE REFUNDED BACK TO THE PATIENT***
Autism Testing at our Seattle (telehealth) testing location has a age limit of 10 years and older, and a age limit of 15 years and older for our Federal Way (in person) testing provider.
For Autism testing, you need to first complete your forms that we will send you through the portal, then call on one of the following scheduling days ONLY between 8:00am-9:30am to be scheduled.
